Please have the following available before beginning enrollment:

• Health insurance information for each child
• Medication names/dosages for each child, if any
• Allergy information for each child, if any

Why do I need to fill out multiple forms per child?
Enrollment for Formation at St. Philip’s requires completed Forms A and B for each child per Diocesan guidelines.  Form A is completed once per year, so if you have already filled out Form A for a child this year, you may opt out during the process.

Forms A and B take a moment to process after you finish each one (you’ll see a pop up asking you to wait).  Please do not close the page until you see a success message or the next form - otherwise it won’t submit the form!
